Adult Services - Adult Day Health Care

Objectives

The primary objectives of the program are (1) to restore or maintain optimal capacity for self care, and (2) to delay or prevent inappropriate institutionalization. The program stress' partnership with the participant, the family, the physician, and the community in working towards maintaining personal independence.

Scope of Services

A multidisciplinary team of health professionals conducts a comprehensive assessment of each participant to determine which ADHC services fulfill the individual's specific health and social needs. This individualized plan of care may include any of the following:

Nursing Services

Physical Therapy

Occupational Therapy

Speech and Cognitive Therapy

Behavioral Planning

Social/Psychological Services

Hot Meal and Nutritional Counseling

Recreation and Social Activities

Cognitive Re-Training

Transportation Coordination to and from Center, if necessary

Individual/Family/Group Counseling

Monthly held Family Support Groups

Eligibility Criteria

The RIO Adult Day Health Care Center Program is designed to provide a warm and supportive environment for the brain impaired adult ages 18 and older, who is experiencing a decrease in functioning primarily due to a brain impairment such as closed head injury, traumatic brain injury, stroke or related condition. The program serves residents of Orange County. Special arrangements may be made to admit eligible people from outside this service area.


Admission Requirements

A medical condition requiring treatment or rehabilitation prescribed by a physician.

Physical and/or cognitive impairments that limit activities of daily living.

A high potential for further deterioration and possible institutionalization if Adult Day Health Care were not available.

Completion of the Admission Packet and required home visit.

Program Days/Hours

Adult Day Health Care is available Monday thru Friday from 7:30 am - 4:00 pm.

Program is closed on major holidays and the week between Christmas and New Year's.

Rates/Funding Information

Adult Day Health Care Services are reimbursed by Medi-Cal or Private Payment.

The ADHC rate is $80.50 per day.
