Adult Services - Adult Day Health Care
Objectives The primary objectives of the program are (1) to restore or maintain optimal capacity for self care, and (2) to delay or prevent inappropriate institutionalization. The program stress' partnership with the participant, the family, the physician, and the community in working towards maintaining personal independence.
Scope of Services A multidisciplinary team of health professionals conducts a comprehensive assessment of each participant to determine which ADHC services fulfill the individual's specific health and social needs. This individualized plan of care may include any of the following:
Eligibility Criteria The RIO Adult Day Health Care Center Program is designed to provide a warm and supportive environment for the brain impaired adult ages 18 and older, who is experiencing a decrease in functioning primarily due to a brain impairment such as closed head injury, traumatic brain injury, stroke or related condition. The program serves residents of Orange County. Special arrangements may be made to admit eligible people from outside this service area.
